Cancer Plan
- Reconstructive Breast Surgery: Actual Charges.
- Hospital Daily Room: $200 per day, 1-30 days. $400 per day, 31 + days.
- Radiation and Chemotherapy (injected by medical personnel): $250 per day.
- Chemotherapy (self-administered): $250 per drug.
- Comfort Drugs (outpatient): Up to $100 per month.
- Bone Marrow Transplant: $5,000.
- Stem Cell Transplant: $1,250.
- Skilled Nursing Facility: Up to $100 per day, limited to number of days hospital confined.
- Hospice: Up to $100 per day, 1 – 60 days. Up to $50 per day, 61+ days.
- Second and third surgical opinion: Up to $225 per opinion.
- Surgery: Up to $7,500.
- Anesthesia: Up to $1,875.
- Blood and Plasma: Actual Charges.
- Wigs and Hairpieces: Up to $250.
- Ambulance: $200.
- Inpatient Drugs and Diagnostic Testing: Up to $40 per day.
- Attending Physician: Up to $30 per day.
- Prosthetics: (surgical) up to $2,000, (non-surgical) up to $250.
- Transportation: Up to $1,500 coach-class plane, train, bus or 40 cents per mile personal auto.
- Family Member Lodging: Up to $60 per day.
Heart Plan
- Heart Transplant: Up to $100,000.
- Hospital Daily Room: $100 per day.
- Private Nurse: Up to $50 per day.
- Skilled Nursing Facility: Up to $50 per day.
- Surgery: Up to $2,500.
- Anesthesia: Up to $625.
- Blood and Plasma: $30 per unit.
- Ambulance: Up to $50.
- Inpatient Drugs and Medicine: Up to $30 per day.
- Attending Physician: Up to $40 per day.
- Electrocardiogram: Up to $100 per period of confinement.
- Oxygen: Up to $100 per period of confinement.
- Transportation: Up to $750.
- Family Member Lodging: Up to $40 per day.
Accident Plan
- Physician Office Visits: $30
- Emergency Room: $300, Child $200
- Hospital Daily Room: $300 per day
- Intensive Care: $600 per day
- Ambulance: Ground $150/Air $1,000
- Blood and Plasma: $100
- Physical Therapy Visits: $30
- Medical Imaging CT, MRI and EGG exams: $150
- Medical Appliances/Crutches/Wheelchair/Walker: $100
- Prosthetic Devices: $500
- Transportation: $400
- Family Lodging/Hotel/Motel: $100 per night
- Lacerations: up to $300
- Burns: up to $900
- Injuries requiring surgery: up to $900
- Dislocations: up to 2,200
- Fractures: up to $2,400
- Paralysis: up to $6,250
- Accidental Death Life Insurance: Employee $50,000, Spouse if on policy $50,000, Child if on policy $12,500
- If Accidental Death from Motorized vehicle or pedestrian: $75,00 (Insured or Spouse if on policy), $18,750 (Children if on policy).
- If Accidental Death from Common Carrier: $100.00 (Insured or Spouse if on policy), $25,000 (Children if on policy)
